Website Process

There are 3 simple steps to providing the information we need to get your Practice website on the Internet. Only step 1 is required for us to get started. Once we have your initial website design you can complete steps 2 and 3.

Step 1.

Complete the physician initial website information form.

To complete this form you will need the following:

  • domain information, where you purchased, login and password
  • if you have not purchased a domain, please provide us the name you want and we will purchase for you
  • provide up to 3 email addresses @yourdomain.com (e.g. drkirsten@foxvalleyphysicians.com)

Step 2.

Complete your personal Physician bio form. All of the information you need to complete this step is at the following link:

Step 3.

Prior to completion of your website you will have an opportunity to review and revise the existing content. Please provide any changes on the following form:
